Found at an 1840s homestead, its solid brass, heavy, has pullies at both ends that still work. Identical on both sides. Maybe for curtain strings/clock chains?
I was going to say clock, but usually the chain or cable winder goes over a sprocket or drive shaft mounted sheeve. I have no clue. Very cool find for sure.

totally agree, it was heavy, pullies still worked, definitely from the American Industrial Era....they overbuilt, and it was made to last. This thing could be used today even after being in the dirt over a hundred years, try finding something like that at Lowe's or Menards.
